Making an appointment with the right counselor is easy.

Make An Appointment

Call the main office (770) 898-2966 and speak with a receptionist who will ask you basic questions to help connect you to the counselor that is best for you. The counselor will call you directly to set the appointment.

Please be sure to complete the appropriate paperwork below and bring it to your first session.

If you are an adult making an appointment please complete the following forms and bring them with you to your first session:

Adult Intake Forms (Age 18+)
Financial Policies & Agreement for Professional Services (All)
HIPPA Policies (All)

If you are an parent or guardian making an appointment for a child or adolescent, please complete the following forms and bring them with you to your first session:

Child/Adolescent (Age 17 and under) Intake Forms
Financial Policies & Agreement for Professional Services (All)
HIPPA Policies (All)

Please note when scheduling children/adolescents that we take a family approach to counseling and need information on parents/guardians to assist in providing quality care for each family member involved.
